Wrong name
My Magikarp evoled into a Gyrados. But when my Gyrados fainted, it said Magikarp fainted.

This one isn't a bug. When it says your Pokémon fainted, it uses their nickname. By default their nickname is their original Pokémon name. This doesn't change when the Pokémon evolves. If you want to change your Pokémon's nickname, go to your profile, click on the picture of your Pokémon and click on their nickname link.
